You will probably want to get the pamphlet "Amateur Construction of
Schupmann Medial Telescopes" (64p.) by Jim Daly from the guys at Stellafane.
I bought my copy there last year for a few bucks. In there is the formulas
for the corrector:
R1 = C/(1+.5(O/C)+(.5(O/C)/(N-1))
R2 = C/(1+.5(O/C))+t
R1, R2 are the surfaces, O is the objective focal length, C is the corrector
focal distance from the first surface and t is the thickness of the
corrector.
I will note that the pamphlet is a lot more complete as to the design of the
telescope than just this and I'd suggest finding a copy, hopefully from
somebody at Stellafane.
