I'm doing a couple like yours right now. Mine are 8"x0.7" and 14.7"x 0.8" (the ones you see on eBay on a regular basis). If you can I highly suggest getting it pre-generated, it'll save you a lot of time. After that any normal tool will work. On my 8" tool I used glass squares from JoAnn's fabrics that were designed to be decorative itmes. They worked fine, as would flattened marbles, or regular old tiles. 
I have found that the grinding difference between pyrex and fused quartz  is not as great as I was led to believe before I started. I'm working these by hand so I'm not as accurate as a machine maker, but I can't tell any difference in grinding speed up through 9 micron AlO2... that's as far as I've got so far. Polishing starts next week.
