There are things that can be done at microwave frequencies where
the wavelength is in the inch or so region.  Try doing that same
stuff at the wevelengths of light and you'll be having to deal
with moving atoms around (small ones may work) just to get that
inversion at a particular frequency.
I'll note that these are transmission rather than reflection
devices so a 21cm refractive lens is a bit on the large side.
BTW, I'd like to find some detail in the construction of the
devices.  All that I've seen have been announcements.
