I recommend United Lens (http://www.unitedlens.com/Default2.htm). 
They have precision annealed molded Pyrex blanks (4"-12" dia.) at good 
prices.  What I have wanted has always been in stock.  (There is a 16" 
molded blank, but it's 3" thick and pricey.)  Best to order machined 
blanks above 12" in diameter, IMO.

You have to call them.  1-508-765-5421.  They will fax quotes for 
larger blanks to you.  You can request an information packet.

I find them to be responsive and easy to work with.  They ship the 
molded blanks promptly.  You can pay with a credit card.  They haven't 
disappointed me yet.
