Well, weren't the lenses themselves OK, but there was a burr or
damage in the lens cell which caused incorrect spacing?.

Either way, not checking its figure by a second means and
actually not looking through it is unforgiveavble IMO, however
difficult that would have been in practice. It also suggest that
they didn't check the null lens assembly before use, too ;)
