Back off a bit on the W and the length of the stroke.  What is happening is
that you are digging the center out too fast and you haven't had the rest of
the surface catch up.
I'll also note that this is one of the ways of getting TDE on a mirror as
the center widens out at that short ROC and gradually the edge gets more
radically off from the center and you have TDE.
