 Yes, there is a forth mirror, but it is a flat.  Except for reversing the
image, a flat does not affect the performance of an optical system.  So I
don't know what you mean by "make all the difference in the world".

It isn't.  You need a fourth reflection to get the image out to where you
can see it without putting your head in the way.  (Assuming that I have read
the OSLO stuff correctly, that is! Of course, that fourth reflection is
going to make all the difference in the world.)
