12 inch subdiameter lap on a 16 inch, and 5 inch radius zone is low, 7 inch
zone is high, and edge is very low.

You basically do not have enough correction in the mid-outer zones.  Do an
off-center stroke by a couple of inches and stroke from edge to edge such
that the lap overhangs the edge by an inch on each side.  Try this for 10
minutes and see if you discern a difference.  Sounds like you are not going
far enough off-center as you stroke through the middle part of the mirror.
