Considering that the book has the PCB layouts in it, I would suggest that
you grab a PCB layout program and do a layout yourself!  The process isn't
hard to do, especially if you are just copying the board.  If you want a
surface mount version, let me know and I'll probably make one up but you
have to remember that the CCD chips are obsolete and the software driver for
that camera isn't in the public domain although it wouldn't be hard to do
the software for another chip.  Doing a library for QB4.5 wouldn't be that
hard to do and I have done one already.
