I think I know where the problem is. You should do it in the following
sequence:

1. Pour the pitch on the tool. it should be thick enough (0.5" thick
won't hurt for you will trim the excess later).

2. When the pitch has stopped flowing, let it harden for a few minutes.

3. Place your mirror on the pitch tool. Before placing it be sure that
you apply a polishing mix on the mirror and the tool, or, cover the
pitch with plastic (Saran wrap ?) then press the mirror on the tool (to
make the tools surface matches the mirror's surface). Press it hard.

3. Take the mirror off the tool,  trim the pitch tool and channel it.

4. After channeling and trimming it, hot press the tool again. Now,
after the hot press check again if there are channels closing. If there
is, trim it again.

5. Now you're ready to polish your mirror. Before each polishing session
you've to be sure that there's a good contact between the pitch and the
mirror that's why you should always cold press or hot press the mirror
before polishing.
