I've seen the S shape on fully polished out mirrors.  I've even got one
example of the problem with a mirror that started out fully polished but
spherical and the guy tried to parabolize it but got the problem instead.
Since the mirror is a 4.25" one, it's kind of hard to see what he's doing
wrong as the mirror is fully enclosed by his hand.
