I found a way to minimize the wrinkling of a plastic sheet used
between the mirror and the tool you are pouring.  Make sure the mirror
is level so you don't get a wedge shaped tool.  

  First thing I do is to wet the ground mirror surface with water.  You
want it fully wet with no puddles of water in the center.  Now your
plastic sheet should stick to the surface or the mirror without trapping
much air.  Put your "dam" around the mirror's edge, and watch out that
you don't pull the plastic too tight.  

  The second thing is to pour your mixture in the center until the
plaster or concrete gets near the edge.  This should push any air away
from the center of the mirror.  Now try to push any air bubbles out to
the edge with your fingers, they may flatten out as the air goes out the
side.  If a bubble won't go away I take a needle and pop a small hole in
the plastic, then squeeze the air out the pin hole.  I have not had any
trouble with my mirror getting stuck to the plaster tool because of the
pin holes.

  I have filled in wrinkles and holes in my tools with epoxy glue. 
There is also a two part epoxy paint that works well to seal up a tool. 
I found it at Lowe's - a big hardware store where it is sold to refinish
bathtubs.
