ATM Worms and gears

I've been following the recent threads about steppers with much interest. 
 But I have a couple of questions about worms and gears.  Do most of you 
making your own mounts buy the worm and gear assemblies, or do you machine 
them yourselves?  Is this something that could be accomplished without a 
great deal of expensive equipment?  And finally, if this is something that 
could be accomplished by an amateur, does anyone know of a good reference 
on how to do it?
