Here's a file. Several things you have to do. Select "Lens" and "Lens 
Drawing Conditions", and change the box "Final dist" to "Draw rays to 
image surface".

Next, get rid of surface 5 - it's not needed.

Next set the thickness of Surface 4 to "Solves" "Axial ray height" "0".

Next set the coordinates on surface 4 to "TLB" "0" (instead of 45, only 
TLA should be tilted to 45 degrees), and set "Tilt and bend" to "Yes".

The result is below.
