The tool glass isn't thick enough.    I'd want to have glass that
is at least .5" thick.
FWIW, I just cast a tool after forming the curve on the glass and
put pads on it to do the polishng of the glass.  I first ground
the glass down to 5 micron flat against another piece of glass
before putting it on the pan and doing the polishing.  Your curve
is really that fine for the corrector surface.  Most of the curbe
is the spherical curve generated by cacuuming the other side of
the glass.
