Why not find a large glass shop which has a water jet cutting machine? These things will cut glass any shape you want up to two inches thick. The edge finish is good enough to leave.

As far as gluing two bits of 19mm, there are usually too many problems with differential expansion of the glue and glass from what I've heard. I'm told by my glass shop that you can get float glass to 38mm thick in the USA. Even 25mm glass would be fine for a 12 incher.
