Jonathan, might try a window...  I ordered one a few years ago for a CCD camera and it was pretty reasonable.   If I can find the link will pass it on.  

If you have to order from a glass supplier, and you are grinding both side, just order it "fine ground"  ie- "Plano- Plano, fine ground."  Saves the supplier a lot of time and money if he doesn't have to polish it.
