[APML] Custom Ring

I'm looking to get a ring made to hold one end of my 300mm f/2.8 lens.

I need it to be  5.5 inches ID with a 2 inch base and 4.25 inches between 
the center of the hole and bottom of the base.

Anybody know anyone who make custom rings?
