[APML] Layer Mask Help Needed

Hello,
    I'm trying to learn how to make a highlight mask using Jerry's book but am obviously missing something.  Here's what the book says...
Procedure:
  1. Select > All. Edit > Copy. Open the original image. Copy it to the clipboard.

  2. Edit > Paste. Paste the original as a layer on top of itself.

  3. Layer > Add Layer Mask > Reveal All.

  4. Alt Click on the layer mask icon in the layers palette.

  5. Edit > Paste the original into the layer mask. It will turn into a black and white image.
No matter what I do I can't seem to get a black and white image.  I start with my working image and do step 1, 2, and 3.  What should I be clicking on in the layers palette in step 4?  Did I miss something in steps 1-3?
