Becouse Photoshop is limited to 8 or 15bits accuracy,
each process you apply to the image problably will
cause the merging of some gray levels. That means that
you are losing information, wich is unrecoverable. Try
to do all the changes in just one simple curves or
levels adjustement.

Yes. If you perform several aggresive changes, it can
introduce a several data loss, that will be reflected
in a posterization of the image. So, you will see
steps instead of smooth gradients.

When working with PS, try to always use the
adjustement layer for curves or levels changes. Be
aware that only PS CS works with such layers at
"16bits" (it is really 15 bits).

If you use any software with 32bits per channel
accuracy (like PixInsight LE), you can forget that
issue and perform almost all the adjustements you
want, without taking care of the rounding error.
