Gary,
Dreamweaver or Adobe GoLive are the 2 best programs available. I personally
use GoLive, it's an easy match with Photoshop. You can use preset scripts or
write you own code. There is a learning curve to both, but not overly
difficult. Each can be purchased for around $150 or so. I would guess that
Dreamweaver is the most popular, but that's just a guess.

They have several different plans. For about $10/month, you get 500 MB
