Mike,

I'm using some 114mm rings from Parallax.  I had to line them with some gasket material to tighten the fit, but I got them to work.  I think he will drill the hole pattern you need to attach your guide scope to the top. I'm using mine on top of a 155 so I can guide with one and shoot with the other.  I think it's finally working now.
