For many US locations, particularly here in Calif, an excellent ISP that provides
such filtering is Sonic.net. They offer a Unix shell as well as PPP, and have a
web-based anti spam tool wrapped around Spam Assassin. They agressively go after
spammers, as well as provide extensive virus filtering which the user can opt to
use or not. I switched to Sonic from Netcom before it was bought by
Earthlink--thats about 5 years ago. Spam on Netcom had gotten overwhelming and has
become worse with Earthlink despite their advertising claims to the contrary.

My Incoming spam averages less than 2 per week despite the fact I belong to 26
Yahoo Groups.
Insert all the usual disclaimers about not being associated with Sonic, etc.

I am a very happy Sonic customer!
