I agree that blurring is likely the best way to deal with very grainy images.  I've gotten some good results with SGBNR (better than with NI).  The only problem that I've encountered is the narrow banding that occurs around bright stars due to the mask.  I've been able to control this somewhat by adjusting the mask but I can't eliminate it completely without significantly reducing the amount of blurring.  There seems to be a definite trade-off between distinct stars and reduced grain.
 
OT: A bit of information I obtained from the product manager of VisInf, if you are a member of the National Association of Photoshop Professionals, you will be able to obtain Grain Surgery at a 20% discount.  In their review of the software, Photoshop User magazine mistakenly listed the price at 159.00.  VisInf decided to honor this.  There will be an announcement in their web site as well as the next newsletter. 
 
But now it looks like there will be further revisions in SGBNR - and it's still free?!

    One of the only and most effective ways to really combat a very heavy grain pattern is to make a masked Gaussian Blur of the image... even Grain Surgery has a problem with clumps of grains... again thinking that this is part of the image and not noise. The masked Gaussian Blur technique that Mike Cook introduced to us attacks everything uniformly except what has been masked off... I use PW for this...
