David, yes it is done at the film development stage. Essentially they leave the film in the developer longer and it creates the impression the film was faster than it says on the box. This will raise the grain slightly but some say that E200 for instance actually IMPROVES with pushing. Furthermore, I'm told your slides will scan more easily if you push the processing. Lastly, a one stop push isn't really equal to doubling the speed so you can push two stops and that is, again I'm told, closer to doubling the speed.
 
I've pushed the E200 I shot and it did not suffer.

I’ve read where people say they “push” their film one step.. What is meant by that? Is it something done at the developing lab i.e. developing 200 ASA film at 400 ASA?
