IIRC, I was a bit puzzled on this as well.  I have a mid 80's vintage C8
which I put on a G11 last year, so yours should work OK.  Mine was on a
Super Polaris (GEM) mount so I can't help you with the fork mount removal,
but I seem to recall that it only took one screw at the top of the scope
and two screws at the rear to hold the dovetail blocks on, then the
dovetail screws into this with four screws at the corners.  You have to
remove the existing screws from the scope and use the Losmandy supplied
screws.  I'm recalling all this from memory (which ain't so good these
days) so I'll double check when I get home tonight and repost if I've said
anything amiss.
