[APML] Grain Reduction?

What is the best was to minimize grain in single
exposures? (I don't yet have multiple shots to
composite.) When I play with the transfer curves to
try to bring out faint detail, the grain comes up
along for the ride. Is SGBNR what I need, and if so,
any tips for how and when to use it? For instance, 
do I apply it at the very end, after all the other
processing steps? Or near the beginning, before 
I've amplified the grain (noise)?
