Alan,
You can minimize the picket fence histogram if you do the majority of
your image enhancement with your scanner's software.  Jerry Lodriguss
has a description of what you observed at
<http://www.astropix.com/HTML/J_DIGIT/COMPEXP2.HTM>.  I can't help you
with a step by step description of Photoshop techniques -- I'm still a
hack after five years.
